Have you received outstanding support?

Our Customer Reference Group is inviting Therapy Focus customers and the people in their support networks to acknowledge Therapy Focus employees who have gone above and beyond by nominating them for our 2024 People’s Choice Award.

The annual award aims to recognise exceptional service delivery and celebrate Therapy Focus employees who demonstrate our Guiding Principles in their work.

If you have received outstanding support from your therapist, therapy team or another employee, you might like to nominate them for the award.

Who can submit a nomination?

Nominations are open to anybody who has accessed Therapy Focus services, and those who have worked with a therapist or therapy team. This includes:

  • People accessing therapy services
  • Parents/Guardians and family members
  • Colleagues from other service providers
  • Health professionals

How do I nominate?

There are four ways you can nominate:

1. Fill out the nomination form online

2. Make a video or voice recording answering the questions below and send it to [email protected]

3. Download a nomination form to print and fill out, or request that a hard copy be mailed to you

4. Call us on 1300 135 373 and book a time to speak to someone over the phone or in person (you can request a translator if needed).

Nominations close at midnight on Sunday 12 May, 2024.
